Insurance and Self Pay

In order to mitigate costs for our clients we participate with many insurance policies either by being in network with them or using a third-party to bill. Insurance companies we work with are:

• Aetna

• United Healthcare

• Cigna

• BCBS (varies by plan)

We bill for the visits but we can not guarantee that there will be no out of pocket cost to you. Depending on individual deductibles and policies there may be out of pocket costs. We will do our best to estimate costs prior to your appointment, but ultimately we do not know until the claim has been submitted. Speak with your provider at the time of booking for more information.

If your insurance is not covered there are self -pay options. Payment is due at the time of service. At your request, we will provide you with a superbill to submit to your insurance company for reimbursement. Reimbursement is not guaranteed by the insurance company. We recommend contacting your insurance company prior to receiving services to determine coverage and reimbursement policies/process. Check with your insurance company to see if they require pre authorization.

Out of network? This is most often seen with some BCBS plans. Request “in for out” of network coverage or “gap exception” from the insurance company. This allows your insurance company to cover the services as “in-network,” to fill the gap they have in their own network. Clients pay the provider at the time of service and a superbill is provided to submit to insurance.

HOME VISITS

Having an office allows us to see many more clients and get our families care ASAP. However, we understand that sometimes clients prefer a visit in their home. We offer home visits on a limited basis depending on client preference and travel distance.

Convenience fee

A $50 convenience fee for home visits is due at the time of appointment.

Travel fee

If a visit is more than a 30 min drive an additional $20 fee will be charged. You will be notified of this fee at the time of booking.
